1 · What AEO is, for a medspa

SEO targets Google’s blue-link results. AEO targets the AI answer itself: the paragraph a patient reads on ChatGPT before they ever scroll, or the AI Overview block at the top of a Google SERP that resolves the query without a click. Visibility is the share of patient-intent queries where one of these surfaces names your medspa in its answer.

Two things make medspas different. First, query intent: patients ask for clinics by treatment (“best botox in Sunnyvale”), by location (“Sunnyvale medical spa”), by trust signal (“safest place for botox”), and by comparison (“Skin Refine vs Allure”) in roughly equal measure. Second, answer compression: AI surfaces almost never return a long list for a local medspa query: they pick three to five names per intent and hand them to the patient as if they were the whole market.

2 · Why medspas are exposed

Most medspa marketing budgets are pointed at Instagram and Google Ads. Both still work. Neither shows up when a patient opens ChatGPT to ask which clinic to book. The audiences are starting to overlap: younger aesthetic patients query AI surfaces before they query Google for service decisions, and the behavior is moving up the age curve.

Because the AI answer is compressed and named, a medspa that ranks 6th organically on Google may rank 0th on ChatGPT: the surface just does not mention them. There is no equivalent of page two. You are in the answer or you are not.

4 · What changes when AEO is working

When AEO works for a medspa, three things move. Mention rate rises: the clinic gets named in a higher share of patient queries. Rank position improves: the clinic moves from being named fifth to being named first or second when the surface compresses its answer. And citation rate rises: the surface starts linking the clinic’s own URL as a source, which compounds because every AI surface treats its prior citations as a signal for the next answer.
