Google AI Overviews optimization for medspas

Google AI Overviews is the AI-generated summary block at the top of a Google search, measured separately from Gemini because the surface, ranking factors, and citation behavior differ. It is also the surface where medspas are least visible of the five we audit. This page explains how it decides, why almost no clinic appears, and what we measure.

1 · What Google AI Overviews does when a patient asks about a medspa

Google AI Overviews sits at the very top of a normal Google search, above the blue links, and answers the query before a patient scrolls. Unlike the other four surfaces we measure, it rarely names a specific clinic at all for a medspa question. We read the actual Overviews in our first study: for medspa questions, Google’s AI Overview tends to answer generically (typical price ranges, what to expect) rather than recommend a business by name.

Across our first study of ten South Bay practices, the local clinic appeared in under 2% of Google AI Overview answers, and eight of the ten practices were absent entirely.

2 · How Google AI Overviews picks who to recommend

Google AI Overviews is served from the main Googlebot index. It has no separate opt-out token the way Gemini does with Google-Extended, so Googlebot is the relevant crawler signal for this surface, and Google’s own guidance treats robots.txt directives for Googlebot as the control for whether a page can appear here at all.

Because so few clinics are named regardless of what they do, the retrieval and authority signals we track on every surface (a retrievable treatment page, consistent name-address-phone details, third-party citations) still matter here, but they compete against a surface that is simply reluctant to name any specific business for this category of question yet.

3 · Why most medspas do not appear

This is the starkest number we measure. In our first study, the local clinic appeared in under 2% of Google AI Overview answers, and eight of the ten South Bay practices were absent entirely. But this is not the clinics uniquely failing: we read the actual Overviews, and for medspa questions Google’s AI Overview tends to answer generically and names very few specific clinics for anyone.

The box most patients see first, ahead of any other AI surface, rarely recommends a medspa at all right now, which makes it an opening rather than a dead end: nobody is winning it yet.

4 · What we measure on Google AI Overviews

Google AI Overviews does not appear on every search, so its slice of the query basket is narrower than the other four surfaces’. Of the 162 total query-runner combinations in a current audit, 18 cost and consideration questions (how much a treatment costs, whether it is worth it) are routed only to Google AI Overviews, alongside the 36 patient query types that run across the four chat surfaces (144 combinations). In our first study, Google AI Overviews ran on 16 of the queries used at the time, since Google only shows an AI Overview for some searches.

We record the same three things per query as every other surface: whether the medspa was mentioned, whether it was cited as a source, and its rank position among any competitors named. These roll up into a Google AI Overviews-specific visibility score (0–100) that sits alongside the scores for ChatGPT, Claude, Gemini, and Perplexity. 5 · The fix patterns that move Google AI Overviews rank

We do not chase surface-specific tricks. The audit ships the same fixes ordered by projected lift, not by activity volume:

  • Confirm Googlebot is not disallowed in robots.txt or challenged by CDN bot management. Google AI Overviews cannot show a page it cannot crawl, and it uses the same index as ordinary Google search, so getting this wrong risks organic search too.
  • Treatment pages with explicit, retrievable copy: one page per major procedure, with the procedure name, location qualifier, and price band stated plainly.
  • LocalBusiness, Service, and FAQPage structured data on the site, kept consistent with the prose.
  • Name-address-phone consistency across Google Business Profile, Yelp, and any third-party directory that covers the metro.
  • At least one third-party citation per major treatment line: a local press piece, a treatment guide, or a credible directory listing.

6 · Google AI Overviews vs the other four surfaces

Google AI Overviews is one of five AI surfaces we audit, and it needs its own line despite sharing a parent company with Gemini. The two are functionally different products: Overviews is embedded directly in Google’s search results and pulls from a different citation pipeline than gemini.google.com’s standalone assistant. Folding them into one score would smooth over a real visibility gap, since a clinic can be well represented on Gemini and still invisible in Overviews, or the reverse.

  • ChatGPT (OpenAI) is currently the largest AI assistant by user volume and issues its own retrieval pass, naming three to five clinics with web tools enabled.
  • Claude (Anthropic) weighs well-structured sources and reasoning chains, and tends to hedge when they disagree.
  • Gemini (Google) sits on top of Google’s knowledge graph and local business data, a source mix distinct from Overviews.
  • Perplexity (Sonar) is a web-search answer engine that leans heavily on Reddit and other UGC for local queries.

We measure ChatGPT, Claude, Gemini, Google AI Overviews, and Perplexity together, because a single surface, especially this one, gives a badly incomplete picture.

    No. Two of the practices we audited rank #1 in their local set. One still appeared in 0 of 16 Google AI Overview queries in our first study. Being the best-known clinic in town is not the same as being the one this surface names.
