Perplexity optimization for medspas

Perplexity (Sonar) is a web-search answer engine that leans heavily on Reddit and other UGC for local queries, a different citation profile from the other four surfaces we audit. It was added as our fifth audited surface in June 2026 once it became a primary answer engine for local discovery. This page explains how it decides, why most medspas do not appear, and what we measure.

1 · What Perplexity does when a patient asks about a medspa

Perplexity is a web-search answer engine (Sonar), not a general chatbot with search bolted on: search and citation are its core product. A patient asks it a local medspa question, and like the other four surfaces we audit, it can name a short list of specific clinics rather than returning a page of links, drawing especially on Reddit and other user-generated content for local queries, a different citation mix from the other four surfaces.

Across the ten South Bay practices in our first study, Perplexity named the practice in about 19% of answers. The engines disagree with each other more than owners expect: one clinic in our sample appeared in 89% of Perplexity’s answers but only 13% of Google AI Overviews’, which is exactly why a single-surface check would mislead you.

2 · How Perplexity picks who to recommend

Perplexity runs two named crawlers. PerplexityBot indexes pages ahead of time, the way a search engine builds an index. Perplexity-User performs the on-demand fetch when a specific user’s live query needs your page. Both are treated as citation-gating: if either is turned away, Perplexity cannot cite what it cannot read.

One wrinkle is specific to Perplexity: its own documentation states that Perplexity-User, the live per-query fetcher, often ignores robots.txt because a user directly requested the fetch. So a clean robots.txt result for Perplexity is directional, not a guarantee, in a way it is not for the other four surfaces.

3 · Why most medspas do not appear

In our first study of ten South Bay practices, Perplexity named the typical clinic in only about 19% of answers, our lowest of the four conversational surfaces (Google AI Overviews is lower still, but it is a different kind of surface). The same failure modes recur: no retrievable treatment page, absence from the metro’s third-party directories and the Reddit or UGC discussion Perplexity leans on for local queries, disagreeing name-address-phone details across listings, and no structured data confirming the page’s claims.

It is a small, early sample (ten practices, directional, not the last word), and Perplexity’s retrieval layer changes with the live web, so a single snapshot is not a verdict.

4 · What we measure on Perplexity

Every audit runs the same 36 patient query types (expanded into 162 query-runner combinations across all five surfaces) against Perplexity. For each query we record three things:

  • Mention. Was the medspa named in Perplexity’s answer?
  • Citation. Was the medspa’s URL or property cited as a source?
  • Rank position. Ordinal position among named competitors in the answer.

These roll up into a Perplexity-specific visibility score (0–100) that sits alongside the scores for ChatGPT, Claude, Gemini, and Google AI Overviews in the combined audit. Methodology detail, including dedup rules, is on the methodology page.

5 · The fix patterns that move Perplexity rank

We do not chase surface-specific tricks. The audit ships the same fixes ordered by projected lift, not by activity volume:

  • Confirm PerplexityBot and Perplexity-User are not disallowed, understanding that a clean result here is directional rather than a guarantee, since Perplexity’s own live fetcher is documented to sometimes ignore robots.txt.
  • Treatment pages with explicit, retrievable copy: one page per major procedure, with the procedure name, location qualifier, and price band stated plainly.
  • LocalBusiness, Service, and FAQPage structured data on the site, kept consistent with the prose.
  • Name-address-phone consistency across Google Business Profile, Yelp, and any third-party directory that covers the metro.
  • Reviews that mention treatments by name, and visibility in the third-party UGC (Reddit threads, forums, review sites) that Perplexity leans on for local queries specifically.

6 · Perplexity vs the other four surfaces

Perplexity is one of five AI surfaces we audit. The other four have different ranking logic:

  • ChatGPT (OpenAI) is currently the largest AI assistant by user volume and issues its own retrieval pass against the live web for local queries.
  • Claude (Anthropic) weighs well-structured sources and reasoning chains, and tends to hedge when they disagree.
  • Gemini (Google) sits on top of Google’s knowledge graph and local business data, a different source mix from the others.
  • Google AI Overviews shares some retrieval surface with Gemini but runs its own citation pipeline off the main Googlebot index, and names far fewer businesses than any of the other four.

A medspa can rank well on Perplexity and poorly on Google AI Overviews, or the reverse. The audit measures ChatGPT, Claude, Gemini, Google AI Overviews, and Perplexity together, because a single surface gives a badly incomplete picture.

    A Google Business Profile is a Google record. It says nothing about you anywhere else, so if it is the only place your details appear, there is nothing outside Google corroborating them to Perplexity or any other surface. We have not measured how any assistant weighs one source against another. What we can say is that a listing with no third-party citations and no on-site structured data leaves Perplexity, and anything reading the page, with a single unverifiable source.
